Community Health Improvement Plan (CHIP)

The Community Health Improvement Plan (CHIP) is a bold and strategic step forward in our collective effort to strengthen the well-being of Summit County. Grounded in data and shaped by community voices, the CHIP is a powerful tool for advocacy, investment, and impact. It identifies forward-facing public health needs and outlines a compelling, evidence-based case for action. This plan equips us to discuss the need for ongoing funding, implement programs that drive meaningful change, and champion solutions that reflect the real experiences of our community members. Summit County is ready to lead with purpose, and the CHIP is our roadmap.

CHIP Overview and Process

Summit County’s Community Health Improvement Plan (CHIP) is guided by the 2024 Community Health Assessment (CHA) and developed in partnership with community organizations, local leaders, and public health staff. An independent facilitator led a two-day workshop where partners reviewed data, shared perspectives, and identified priority areas through discussion and voting.

The CHIP focuses on key community conditions—such as housing, education, transportation, food access, and income—that shape health and well-being. Health equity is central to the plan, with strategies designed to reduce barriers and promote fair access to opportunities across the county.

Together, the CHIP provides a five-year roadmap to align partners, guide action, and track progress toward a healthier Summit County.

CHIP Priorities and Workplan

The 2024 Community Health Assessment (CHA) helped shape the priorities in this CHIP by highlighting where health challenges remain in Summit County. While the county performs well overall compared to the rest of Utah, the CHA identified gaps related to access, affordability, and equity.

In response, the CHIP focuses on three key priorities identified through community input and partner collaboration: strengthening partnerships to address the social and economic drivers of health, improving access to healthy and affordable food, and increasing immunization coverage. Together, these priorities reflect Summit County’s commitment to prevention, equity, and coordinated action to improve health for all community members.
