About Clinical Services Programs

The Public Health Nurses (PHN) at the Summit County Health Department are here to ensure that everyone residing in Summit County has a trusted source to rely on for clinical services, knowledge, and resources. Our focus is to help prevent diseases from happening so that you can live your best life. If communicable diseases occur, we will help to educate those involved and work to prevent further spread within our community.

Immunizations & Vaccinations

Immunizations are one of public health’s greatest triumphs and an important aspect of preventative medicine—appropriate for all ages. We are committed to providing affordable vaccines to our clients—free to low-cost options are available to those who qualify. If you have insurance you can check here to see if we accept your policy.

Immunization Records

Here is a link with instructions for obtaining your immunization records. You can come into any health department location to request a copy of your vaccine records. However, there is a small fee for an official copy.

Immunization Exemptions

A parent/guardian may claim an exemption to immunization for medical, religious, or personal reasons, as allowed by Section 53G-9-303 of the Utah Statutory Code. To receive a vaccination exemption form, the legally responsible individual who claims the exemption for the student must complete the online immunization education module.

Women & Children's Health

Our reproductive services and Breast & Cervical Cancer Program in Summit County help to provide affordable contraception, family planning, and cervical and breast cancer screenings. Our services are confidential and based on income— some services are free if you qualify.

Available contraception options include:

  • Emergency contraception
  • Free condoms
  • Oral birth control
  • NuvaRing
  • Depo-Provera Injection
  • Long-acting Reversible Contraception (LARC) – IUDs (hormonal and hormone-free options) and arm implants

We do not take insurance for this service.

We offer free breast and cervical cancer screening for people who qualify. This program can include pap smears, breast exams, and mammograms. Check here or call us at 435-333-1500 to see if you qualify and to enroll in the program. If you do not qualify for the program, we do offer these services for a fee.

Women who qualify for the B&C Program also qualify for our health screening program (BeWise). This provides women with the knowledge, skills and opportunities to improve their diet, and other lifestyle behaviors to prevent, delay and control cardiovascular problems and other chronic diseases. STIs & Communicable Diseases

Sexually Transmitted Infections (STI) are the most highly reported communicable disease in Summit County, UT. The Summit County Health Department has affordable testing for chlamydia, gonorrhea, syphilis, and HIV. If you test positive we can either provide treatment, or help you obtain help. Our services are confidential and professional. Call your closest location to set up an appointment.

TB testing is routinely needed for some jobs, immigration, or if you believe that you have been exposed to TB. We offer multiple types of testing and can help you through the process if you test positive. Skin tests are only available at Park City or Kamas on Monday, and Park City or Coalville on Tuesday. Call your closest location to set up an appointment.

During spring and summer months there is increased concern about rabies due to the bat population in Summit County. If you believe you might have been exposed you can use this Rabies Exposure Assessment Tool to evaluate your risk and get advice on the next steps to take.

We also offer testing for hepatitis B & C, and HPV. Call one of our locations for pricing and to schedule an appointment.

Why do we have reportable diseases?

According to Utah state law, certain communicable diseases are reported to the health department in an effort to prevent transmission and keep our community safe. A list of the reportable diseases can be found in the link below. The health department investigates, educates, and intervenes when necessary. We work closely with the state, local physicians, and other local health departments to minimize the impact of disease on our community’s health.

You might be contacted by one of our staff if you are diagnosed or exposed to one of the diseases listed in the link below. Please cooperate with them and feel free to contact the health department if you would like to verify their identity.

How to report one of these diseases?

Most of the reportable diseases will automatically come from a lab or physician. If you have knowledge of a reportable disease that you believe has not been communicated with the Summit County Health Department you can call our Communicable Disease Coordinator at 435-336-3224 or fill out this Confidential Morbidity Report

Home Visitation

There are situations that prevent some of our clients from coming into the health department. We have two programs to help when these situations arise.

Homebound Program

When a client is unable to leave their home due to a physical, safety, or mental barrier. We will send out a mobile clinic van to administer immunizations (such as covid and flu) in the home. Clients must qualify through a screening by one of our Public Health Nurses. Call 435-333-1531 if you, a patient, or a loved one qualify.

Targeted Case Management (TCM) Program

Our TCM Program provides in-home assessments and resources for newborn children who are on Medicaid. Our trained Public Health Nurses, Community Health Workers (CHW), translators, and/or Clinical Assistants will help evaluate any needs that the family might have and help connect them with the appropriate resources throughout the county. They enjoy helping to educate the family and help them anticipate and cope with their life change. Call 435-333-1531 if you, a patient, or a loved one qualify. Visits usually last approximately 1 hour.

Below is a list of insurances that we currently accept for vaccines. Call any of our locations if your insurance is not listed—we are constantly trying to expand our abilities to better serve our community.

We currently DO NOT accept Cigna or University of Utah plans.
