Birth Certificates

How to obtain recent birth certificates:

Certificates can only be processed before 4:30 Mon-Thurs.

Birth Certificate cost: $22 plus $10 for any additional copies.

In-Person: Please call to schedule an appointment at any of our health department locations.

You MUST bring valid identification (internal link to Valid ID page) with you to pick up birth certificates. Identification is required for all non-public Vital Records. 

Summit County Health Department reserves the right to request additional forms of identification to verify entitlement to vital records. Immediate family members are required to prove their relationship to the subject of the record.

Examples include-

  •   Parents are listed on the birth certificate.
  •   Siblings have a birth certificate with at least one common parent.
  •   Grandparents have a chain of birth certificates showing relationships.
  •   Children have a birth certificate with the parent whose birth certificate they are    requesting listed.
  •   Individuals may need to show marriage certificates to show change in name trail.

Historical Records:

Certificates of births that occurred in Utah since 1905 can be obtained in the offices listed below.

Birth Certificates are public record after 100 Years of the certificate being recorded.

Death Certificates

How to obtain a death certificate:

Online: Visit here.

In-Person: Please call to schedule an appointment at any of our health department locations.

You MUST bring valid identification (link to Valid ID internal page) with you to pick up birth certificates.

Death Certificate cost: $30 plus $10 for any additional copies.

Historical Records:

Death Certificates are public records after 50 years of the certificate being recorded. These records can be found here.

Non-traditional or home burials should contact Summit County Planning & Zoning at 435-336-3124 or the Summit County Recorder’s office at 435-336-3238.

List of Valid IDs

If you cannot provide acceptable identification, ask a spouse, parent, grandparent, sibling or adult child, who can provide appropriate identification, to request the certificate. Proof of relationship may also be required, such as birth, death, or marriage certificate.

Primary – at least 1 of the following:

  • Alien Registration Card/Permanent Resident Card
  • Temporary Resident Card

  • Weapon or Gun Permit Card

 

We cannot accept Novelty ID Card.
